How to Register

New patients

All UK residents have the right within the NHS to be registered with a Doctor. We are delighted to accept new patients onto our list. We promote continuity of care and all patients will have a named accountable GP. You will be encouraged to see your own GP as often as possible.

Practice Area

We are pleased to register anyone who lives within the practice area. The practice area is bounded by the River Exe on the South, Rydon Lane and Pinhoe on the East, Western Way and Magdalen Street on the West and Wrefords Lane and Cowley Bridge Road on the North.

If you wish to register with the Practice, please visit the St. Leonard's Practice and speak to one of our administrators. Please ensure you know and bring along appropriate forms of ID as listed below:

Full name, including previous names
Date and place of birth
Last address
New address and post code
Last name and address of your previous Doctor
NHS number if known, - not the same as your National Insurance number!
Date you first came to live in the UK (if you have come from another country)
Proof of address, ie a utility bill, bank statement
Photo ID, ie a driving licence, passport

If you are on regular medication, you should also bring your latest repeat prescription. This will help us continue supplying your medicines while we wait for your previous doctor to send us your medical notes.

Please note we do not accept completed registration applications after 5:30pm

Family Registration

The practice has a long history of registering families together with the same doctor. We think this helps our work as family doctors, by allowing doctors and families to get to know each other.

Change of address
If you change address or move outside the area, you may be asked to change practice.
