The PCN pharmacy team works alongside your practice to help and support the practice team and patients.

Our Services Include:

  • Medication reviews
  • Issuing medications
  • Changing repeats
  • Actioning clinic letters and discharges
  • Contacting patients regarding their medications
  • Dealing with more complex medication queries
  • Finding alternatives
  • Checking patient monitoring is up to date
  • Supporting the local NHS with changes to brands and formulations of medications and achieving cost-effective prescribing for the practices
  • Following NHS guidelines and actioning any changes to medications
  • Contacting patients to offer guidance, support, and reviews
  • Responding to safety alerts and medicine recalls

You may speak to a member of the pharmacy team if you request a one-off medication, need a medication review, have a specific query about your medication, or if we need to make a change to or discuss your medications. This is usually via the telephone or via Accurx.

Our PCN pharmacy team consists of Clinical Pharmacists and Pharmacy Technicians. We work with your GP practice, local hospitals, the ICB (Integrated Care Board), care homes, and community pharmacies to help bridge the gap between primary care, secondary care, and the community. This improves communication between the different sectors. Our team is also here to support patients in getting the best out of their medications and helping you to live well.

How to Get in Contact with Us

Your GP practice will assess any requests or queries, and you may be referred to the pharmacy team as part of this process. If you are having regular reviews with one of our team members, we will instruct you on how to reach us. Otherwise, these will be scheduled appointments.
