Each provider requires a patient questionnaire to be filled. These are different depending on the private provider used. You do not require a GP appointment to discuss referral. Please download the correct patient questionnaire, complete this and return via email or in person to the practice. Your GP will then be able to refer on to your requested provider. The waiting times are always changing and these can still be lengthy even through the right to choose. Please note that it is at the discretion of the GP to enter into any shared care when prescribing in ADHD. Unfortunately we often find that the annual checks for ADHD medications often don't occur placing the full risk of the medication unfairly on the GP.
