Completing forms or providing medicals are not included in the list of tasks carried out by a Doctor for the NHS so if your Doctor completes a form or medical on your behalf there will be a charge - see GP Service Fees
The doctors are most likely to complete medicals such as required for Heavy Goods Vehicle, Taxi, Insurance and Bus drivers.

If you wish to make an appointment for a medical, please telephone reception.
If you want a form completed, please leave it at reception, with THE REST OF THE FORM FULLY COMPLETED. The doctors have a busy time and form completion is not high on their list of priorities. Please leave several days before collecting the form. Our usual turnaround time is 28 days and It would be sensible to ring to check before collecting forms.
Please note that the doctors will not generally complete a Passport Application as this form now requires details from their own passports.
